This week Toronto Hydro workers were in the Junction area checking handwell covers. The inspection seems to consists of assuring there are no wires missing a non-conductive cover, or simply old wires no longer attached to anything that could be live.
Sometimes they cover the wire bundle if any with one of the plastic pails in the pictures.
Hopefully after the blitz check they will return to the problem with new practices and methods to completely remove the possibility of this occurring again.
